One thing you will find out is that almost every insurance carrier has coverage for the surgery. The real question is does your policy cover it. Just because they offer the coverage doesn't mean that whatever group you have your policy with, like an employeer, paid for that as an inclusion in your package.

Think of it like car insurance, just because the insurance company offers rental car replacement you may not choose to carry it on your policy. Same thing for health insurance when your employer selects what will be included in their offerings. Best thing to do is call them directly and ask.
